Call 905-688-5550 x3243 or email [email protected] to book an appointment. Please note that some immunizations are not covered by insurance, so there may be a fee.
You can get your immunization records in one of three ways:
- You can ask your caregivers if they have a copy of your “yellow card”. Many caregivers were given this immunization card when you were a baby and brought it to appointments to have it completed by the doctor with each immunization.
- You can check with the Public Health Department in the area where you grew up. They will likely have documentation of your immunizations. If you moved during your childhood, you may need to contact each of the Public Health Departments where you lived.
- You can try contacting your childhood doctor’s office directly to see if they still have your immunization records on file.
There is no cost to seeing a doctor or nurse practitioner for routine concerns if you have a valid OHIP or UHIP. If you have purchased insurance such as Manulife or guard.me, charges for the appointment will be put on your student account, and you will need to submit a claim to get reimbursed.
If you need forms filled in or need specialty services, there may be a fee. We will tell you before receiving these services.
