You may request academic consideration if you’ve missed a test, assignment, or any other graded work due to illness or extenuating personal circumstances.

The student absence self-declaration form may be submitted once per course per term. Submit directly to your instructor by email or within your course site in Brightspace, if this option has been activated, within 24 hours of the end of the absence. A doctor’s signature is not required.

Medical verification form (over 72 hours)

If your medical condition extends more than 3 days, a medical verification form will be required, which must be signed by a doctor. If you need to be assessed by a physician at Student Health Services, please call 905-688-5550 x3243 to make an appointment.

Exam deferrals

For test and midterm deferrals where absence is less than 3 days, please complete the student absence self-declaration form process.

For final exam deferrals, a medical verification form signed by a doctor will be required. Call 905-688-5550 extension 3243 to make an appointment.
