Student Absence Self-Declaration Process

At the February 12, 2025 meeting of Senate revisions to FHB III.C.13.2.2 (Accommodation of students with acute medical conditions or personal events or circumstances) were approved and reflected in the Student Medical Self-Declaration Form, which is now known as the Student Absence Self Declaration Form.

This form, which is used for absences of up to 72 hours, may now be used in cases of medical conditions (including physical or mental health concerns) or personal events or circumstances (e.g., housing insecurity, gender-based and sexual violence) which seriously impair the student’s ability to meet academic obligations. The Student Absence Self-Declaration Form may only be used once per course per term, with additional uses only permitted at the discretion of the instructor.
