All booking requests begin by fully completing and submitting this form.  Booking requests will be examined and requests will be granted based on availability, suitability, and rental terms and conditions. This form will be processed by the facility manager and the requesting party will be contacted to discuss the request within five business days. Depending on the requirements of the event, rental, staffing and equipment fees may vary.
