Continuing Teacher Education Forms

As part of your application to Continuing Teacher Education (AQs/ABQs) programming at Brock, you may need to include an additional forms and documentation to complete your registration.

Step 1: Download

Download the necessary form(s) indicated in the prerequisite section of the AQ/ABQ course your have registered for.

Step 2: Complete

Physically print, fill and scan or digitally complete and sign and save the forms required.

Step 3: Submit

Submit your completed form and other necessary documents.

For Intermediate, Senior, or Honour Specialist courses.

Confirmation of Teaching Experience Forms

Single Board Form

For Part 2, Specialist and Honour Specialist courses.

Principal's Qualifications

For all PQP courses

Waivers

Recommendation Waiver for Prerequisite Courses

For applicants currently completing a prerequisite course at another university.

Recommendation Waiver Agreement for New Grads

For new Bachelor of Education or Diploma of Education graduates.

Agreement for Non-OCT Members

For teachers looking to take an AQ or ABQ who are not Members of the Ontario College of Teachers.
