Coaching Conversation Record Sheet
This record sheet from The Literacy Coach’s Survival Guide: Essential Questions and Practice Answers (2nd ed.; Toll, 2014) can be used to help coaches and teachers keep track of their topics/problems under discussion, the goal that has been set and the intended course of action to meet said goal, success criteria and assessment tools for determining if the goal has been met, and a focus for the next meeting with a list of required tasks that must be completed before then.
