Alternative Format Request Form

  • To be eligible for this service you must be registered with Student Accessibility Services. Requests for alternate format copies of course textbooks should be referred to Student Accessibility Services’ Alternate Format Text page.

    To request library material in an alternate format please complete the following form.

    Please provide a full citation for the item(s) you are requesting: (Book: Title, Author, Edition/Year of Publication, Publisher, ISBN; Journal article: Author(s), Article Title, Journal Title, Volume, Issue, Page numbers.)

    Requests will be reviewed within 2 business days of receipt. We will endeavour to provide you with the preferred format in a timely manner.

    According to Canadian law, “it is not an infringement of copyright for a person with a perceptual disability, for a person acting at the request of such a person, or for a non-profit organization acting for the benefit of such a person to make a copy of a literary, musical, artistic or dramatic work, other than a cinematographic work, in a format specially designed for a person with a perceptual disability” (Copyright Act, R.S. c. C-30, s. 32 (1)(a)). A perceptual disability is defined as a “disability that prevents or inhibits a person from reading or hearing a work in its original format, and includes such a disability resulting from (a) severe or total impairment of sight or hearing or the inability to focus or move one’s eyes, (b) the inability to hold or manipulate a book, or (c) an impairment relating to comprehension” (Copyright Act, s. 2).

  • Terms of Agreement

    1. I am requesting the alternate format material due to a documented print disability.

    2.I agree that the requested materials are to be used for my personal education, research, and/or study only.

    3. I will not share, sell or distribute the alternate format to anyone regardless of reason.

    4. I will delete alternate format files once they are no longer needed for personal research or study.

    5. I understand that the alternate format is provided to me without a guarantee, expressed or implied.
