Housing Services is committed to working with students to provide the accommodations necessary to ensure that all students have the opportunity to live on campus.

Deadline to Submit Request

All Applicants – Documents are due a minimum of 8 weeks prior to your intended move-in date.

Fall Term – Due by June 15th

Winter Term – Due by November 15th

Spring/Summer Term – Due by March 15th

All students seeking an accommodation are required to fill out the Housing Services Accessibility Accommodation Request online through Microsoft Forms every year. In order to ensure your accommodation needs are met, we require this form to be completed at least eight weeks prior to your intended move-in date. For the majority of students, this deadline is June 15th. If you are unsure about your deadline to submit, please contact us at [email protected].

If you are not submitting documentation to Student Accessibility Services that references your accommodation needs in a residential environment, please download our Housing Services Accommodation Documentation Form and complete it with your health care provider. Once the form is complete you can attach it to your Accommodation Request form prior to submission.

Please review the instructions below prior to submitting your request form.

Registration of Service Animals – Students requiring the support of a service animal should review the Animals on Campus policy and the associated FAQs. Once an animal is registered, the information will be shared with our team in Housing Services to ensure appropriate accommodations are assigned.

Emotional Support Animals (only permitted within a student’s assigned residence room) – Students requiring the support of an emotional support animal are required to complete, along with an attesting professional, the Housing Services Accommodation Documentation Form  and upload it along with their Accommodation request. Once the form is reviewed, Housing Services staff will contact the student directly with respect to the additional documentation required.

Students seeking a Medical Meal Plan Exemption: Brock Dining Services are able to accommodate a multitude of dietary needs. Students with dietary concerns which require a medical meal plan exemption ONLY are required to complete this form. For all other dietary concerns, please include any information or restrictions in your residence application on the Meal Plan and Dietary Needs page.

Private Room Accommodations: Residence is a high density living environment with shared facilities and resources. Requests for a private room as an accommodation based solely on a desire or need for a quiet or reduced distraction environment will not be granted. A private room would not be guaranteed to provide for such a space to any appreciable degree beyond living in a standard double room.

Accessibility Tours: As we review requests, we may reach out to students to arrange an opportunity to review their particular needs and tour some rooms on campus.  These tours are typically arranged in May and June.  Students may also request a tour by emailing us at [email protected].

Request Form: Students need to complete the request form each year even if their needs have not changed.

Student Accessibility Services: If you have shared your documentation with Student Accessibility Services, we still require your express consent in the Request form (linked above) in order for your information to be shared between our offices. You do not need to submit additional documentation through this process if you have provided Student Accessibility Services with your documentation (containing recommendations for the residential environment) at least eight weeks prior to your intended move-in date.

Housing Services Accommodation Documentation Form: If you have previously submitted documentation or had accommodations with us, you are only required to upload new documentation if there have been changes to your requirements. Should we require further information or clarification, we will contact you directly. If you have had changes to your requirements please work with your health care/attesting professional to complete the Housing Services Accommodation Documentation Form.

Please note: we do not accept medical or doctor’s notes/letters on their own but you may include them as additional documentation if you choose

Student Accessibility Services: Students who have shared (or plan to share) their documentation with Student Accessibility Services need to provide their express consent in the Request form (linked above) in order for information to be shared between our offices. Students then only need to complete and attach Section One of our Housing Services Accommodation Documentation Form if they have provided (or will provide) Student Accessibility Services with their documentation (containing recommendations for the residential environment) at least eight weeks prior to their intended move-in date.

Housing Services Accommodation Documentation Form: Students requesting an accommodation for the first time and who are not registering with Student Accessibility Services OR whose documentation on file with Student Accessibility Services only includes information about academic requirements, should complete the Housing Services Accommodation Documentation Form in full. Section One needs to be completed by the student. Section Two of the form needs to be completed by an attesting professional who is familiar with your accommodation needs. The form then needs to be attached to the accommodation request. Students can also upload up to two additional documents as needed.
