Letter of Permission form for OSAP
Use this form if you are taking courses at another school under a Letter of Permission, and wish to have these courses added to your OSAP course load.
OSAP Processing Forms
The following forms allow you to make changes or updates to your OSAP application. Completed forms must be submitted to the Student Awards and Financial Aid office no later than 8 weeks prior to the end of your study term. Forms are normally processed within 2 weeks of receipt.
Course load change form
Use this form if you add, drop, or withdraw from one or more courses while receiving OSAP funding.
Use the appropriate forms below if you are changing your student status (married, single dependent, single independent, or sole-support parent). A student income update must accompany all status change forms. Be sure to include all other requried supporting documentation.
Change of Permanent Disability Status
Use this form to declare that you have a permanent disability on your OSAP application, or to remove a prior declaration.
Disability Verification Form
Use this form to obtain disability-related information from your physician or other regulated health care practioner. This is required if you're applying as a student with a permanent disability.
Change to Married or Common-law relationship
Use this form if you become married or common-law during your study period.
Change to Sole-Support Parent or Single Independent Student
Use this form if you become a sole-support parent, or if you end a marriage or common-law relationship while in your study period.
OSAP Application Update: Parental Information
Use this form if you have been advised by your financial aid office that information from your parents is required for your submitted OSAP Application for Full-Time Students to be considered.
It is important to keep your income information as up-to-date and accurate as possible. Use this form to update your income information throught out the academic year. Note that you may also update your income in person at the Student Awards and Financial Aid Officer (ST402).
Use these forms to request a review of your OSAP file based on exceptional circumstances regarding your income, your parent's income, or your spouse's income.
Parental Income Verification: Foreign and/or Non-Taxable Income
Use this form if your parents have foreign and/or non-taxable income which must be verified to process your Ontario Tuition Grant.
Adjust Income Contribution due to Lack of Employment
Request for Review to consider adjustments to the student’s expected income contributions in the pre-study period and from a student’s spouse in the pre-study and study period (if applicable).
Assessment on Parent's Estimated Income
Use this form if your parent's estimated 2015 income is a better reflection of their current financial situation than their actual 2014 income
Assessment on Spouse's Estimated Income
Use this form if your spouse's estimated 2015 income is a better reflection of his/her current financial situation than his/her actual 2014 income.
Use these forms to request a review of your OSAP file based on exceptional expenses that you or your family have incurred during the pre-study or study period. Please contact our office for exceptional expenses not listed below (examples include legal costs, uninsured medical costs, essential home repairs, etc.). Supporting documentation is required, please read the instructions carefully.
Adjust Living Allowance
Use this form if you are:
- A single, independent student who lives with family but pays room and board OR
- A single, dependent student who lives away from home during the pre-study period in order to work full-time, OR
- A single, dependent student who does not live with family during the school year, even though the family home is within 40km of Brock, because it takes more than one hour by public transit, or because public transit is not available for your commute, OR
- A single, dependent student who does not live with family during the school year because the family home is overcrowded or cannot live in parents home due to psychological or physical health reasons
- A single dependent student who does not live with my parent(s) during the study period (supporting documentation required)
Adjust Local Travel
Use this form if you are commuting more than 30km to Brock, or if it takes more than one hour by public transit, or if public transit is not available for your commute.
Other OSAP forms
Download a Request for Continuation of Interest-Free Status (Schedule 2)
This is for full-time students who will NOT be receiving OSAP loan funding for this current academic year and session. It is used to ensure that your previous loans continue in interest-free status. Students who have previously defaulted on their student loans or who have declared bankruptcy, which included OSAP loans, are not eligible for interest-free status. Please send or drop off this document to the Student Awards and Financial AId Office for processing.
You can choose to have your funds transfered directly to your Brock student account or to your personal bank account. You can use this form to change the option you originally selected on your OSAP application.
Students are expected to progress through the successive years of their program of study. Frequent or multiple program switches, dropping from a full courses load to a part-time course load, withdrawing from a program, repeating a program and/or taking programs at the same level of study may also be considered lack of progress and lack of academic direction on the part of a student who is receiving OSAP funding. If you are required to provide an Academic Edit letter, please use the template in the above link.
As an OSAP recipient, you are expected to successfully complete a 60% courses load (1.5 credits per term). You are not expected to maintain a certain average to receive OSAP; you are just required to pass a 60% courses load. Students with a permanent disability (registered with the Student Development Centre) are required to pass a 40% course load. If you are placed on Acacdemic Probation, use the template in the above link to complete your Academic Probation Letter.
Request for a Temporary OSAP Password
Request a temporary password when you have been locked out of the OSAP website.
Note: there is a faster way to get your OAN number and password. Just drop by the Student Awards and Financial Aid Office with your Social Insurance Card and a valid, government issued photo ID. You are also welcome to drop by any other financial aid office at another post-secondary institution that administers the OSAP program to retrieve your OAN number.

We will close for the winter break on Wednesday, December 23rd at 4:30pm. We will reopen on Monday, January 4th.
