As a returning student, you can request that previously approved academic accommodations be continued in the D3 term by submitting this online form.

Select the previously approved academic accommodations that you want to continue in D3. In some circumstances, updated documentation may be required before a request can be considered.

I acknowledge that by submitting this request I am responsible for meeting with instructors to discuss approved accommodations, and making myself aware of SSWD policies and procedures (including but not limited to: Test and Exam Requests, Missed Appointment Policy, and Peer Note Taking Service).
