Payment can be made via cheque ONLY, payable to Brock University in Canadian dollars. Upon completion of this online form you will receive an email copy of your submission. Please include a copy of this with your payment.

IMPORTANT NOTES
Courses are of limited enrolment. We reserve the right to cancel any course for which there is insufficient enrolment. Confirmation of attendance will be sent upon receipt of payment. A refund, less a $50.00 administration fee, may be issued for cancellations if made in writing prior to May 1, 2013.
