UHIP Cards

The University Health Insurance Plan (UHIP) provided by Sun Life Financial does not issue UHIP cards; each student must print his or her own card. Students can now register on the Sun Life website at www.mysunlife.ca to create an account and this will allow each plan member to print his or her UHIP coverage card, to print a Claim Form, to check the progress of a claim and to apply for Direct Deposit of a claim.

In order to register, students will need to provide the following personal information:

Date of birth
Country of residence (Canada)
Postal code (Brock's postal code - L2S 3A1)
Contract/policy number (050150)
Certificate/member ID (BU00 followed by the student’s 7-digit student ID # (i.e., BU001234567)

If there are problems registering, creating an account or printing the UHIP card, there are instructions on the website and also a toll-free telephone number to call Sun Life Financial directly (1-877-786-5433).

Students can also call our office at 905-688-5550 ext. 4785 for assistance during regular business hours (Monday to Friday from 8:30am -12:00 noon and 1:00pm - 4:30pm).
