Forms Applicable to All Students

General Request Form
Please complete a general request form if you require confirmation of enrollment or confirmation of degree completion. All forms require a 5 working day processing time.

Student Information Form
Please make sure we have an up to date student information form on file in the Graduate Programs Office. 

MBA Independent Study Proposal Form
Must be submitted prior to the close on online registration each term.  Independent studies are not available to MBA (ISP) students.

MAcc Independent Study Proposal Form
Must be submitted prior to the close of online registration each term.  This form applies to the required independent studies in the general stream and information systems stream.  MAcc students wishing to pursue an independent study as an elective should use the MBA Independent Study Proposal Form found above.

Application for Graduation
Application for graduation at Fall Convocation (October) must be received in the Office of the Registrar by July 1. Application for graduation at Spring Convocation (June) must be received in the Office of the Registrar by February 1.
